Smoking Guns is the stand-alone version of the Quake 3 mod Westernquake 3.
I don't know how much has been improved for the stand-alone because I am still downloading but the mod version was a very fun game with a cool atmosphere.

It's a quasi-realistic shooter (think: ~Counterstrike. Maybe a bit less depending on how you look at it) which takes place in the wild west as it was portrayed in Leone's westerns. This is more a tribute to italo-westerns than to the wild west.

Hard to say. It's on the Quake 3 engine and while it looks good for that there are also Quake mods that look better, tbh, so if graphics matter a lot to you this is one major downer already.
I don't know how active the community of FoF is and I don't know how active the community of Smoking Guns is going to be, but the mod version of the game was pretty much what most people here would consider dead. There was almost always one populated server in the evenings but that's not too much, is it?
As for the gameplay I'd say it isn't all that different from FoF. They aren't the same game but if you liked one you might easily like the other as well.
The atmosphere of the game (mainly thanks to the music) is very good though and what little community there is is made up of fair players for the most part. The situation is kind of similar to the RO mod days, just not quite as bright. But maybe that's just my pink nostalgia glasses.

Installed it, ran it but there was only one server and it was a test server.

You are doing something wrong then, because since it's out there have always been at least 1 full server around, plus a few empty/near empty ones.

EDIT: Starting at 27:45 is a bit about Resistance and Liberation so you might even want to get it just for that.
EDIT2: Forget about the Resistance Liberation bit. They say practially nothing of interest. Get it for the Smoking Guns section.

The important thing is the mod is now stand alone so I don't require a copy of Quake III to play which I sold ages ago.
