I collected a landmark in fawtick bayou and later noticed that the landmark is shown as not collected on the map. So I returned and the landmark cannot be collected again because the sign is missing. I am pretty sure, I collected it before. I cannot say when the issue appeared because I noticed it way later. This is my first playthrough on PS4 and I died a double of times after I collected the landmark. Maybe it has to do with that.
