What does the hammer and wrench symbol in the server browser indicate?

Custom server.

Does that mean perk progression is disabled? The server is subscribed to workshop game modes but they aren't permanently enabled.

it means Custom server. It does not mean Unranked server.

Look at your overview page when you connect to the server It will say RANKED / CUSTOM RANKED / UNRANKED

But for example with the "Be the Zed" mod it will say RANKED, but you can see in log it says UNRANKED, that is a bug I reported to [TW]Zane maybe TWI can look at that because obviously some mod can break these informations.
