This seems to only happen if I'm actively engaging zeds - if there's no zeds around, this doesn't happen.
I'll alt fire the doomstick, and reload, and immediately after the reload finishes, the doomstick shoots a shell - without me hitting the fire button.
This happens a lot during combat.
I believe a possible cause of this bug is if you have alt-fire mapped to one of the "side" buttons on a mouse, like where your thumb would rest. These buttons are frequently called "Mouse4" or "Mouse5". I have mine mapped to Mouse4.
A second user confirmed he has the same issue, with alt-fire mapped to one of his side-mouse buttons, in this thread:
