The only game I know which a jam is implemented is Far Cry 2.
I know it is really unrealistic there, because it happens too often. But it can happen, and when you need to push the reload button. Then comes a animation in which the jam will be solved.

The stoppage of a gun is imho a must have for every game that wants to be real as possible.

Maybe also something like cleaning and oiling of weapons between the campaign maps, when beeing dead or while fighting.

If it would be most realistic as possible then there must be different jam types such as one feed, double feed, and so on.
