This trophy is bugged for sure, I have completed the game 100%, done literally everything, found all Landmarks, Opened all 3 gates (which could be triggers for this trophy but not in my case), all Crates, all Plates, Evolutions, Missions, etc and it still refuses to pop...I'm not very keen on starting a new playthrough after 18 hours of grinding. Hopefully this gets patched out soon.
As for the crashes, I've experienced 6 in total and all of them were at the loading screen before a story cutscene, game would boot me out, say there was an error with the application and close...thankfully my save is ok but it's kinda scary that this happened this may times.
All this is on PS4, Hopefully this can be resolved soon.
