Ok, so it's multiple people, maybe a DLC map-pack for KF or it could be something not KF. Either way, guy on the right looks kinda chubby and is holding a Machete or chef's cleaver.

In human anatomy, a full length arm reaches round about the thigh area of a person, past the waist at the very least.
The Bloat's arms do not. That is starting to look like a bloat.
That handle is the same shape as the Meat Cleaver's handle too, not the machete's.
