So to adress a few questions here:
1) Server location hasn't been decided 100% yet but the thought initially is to have a UK server and a Chicago, US server. Other options are possibly a Berlin-Denver server combo instead but it will largely depend on demographics represented by the community. I have no problem with having a Kiwi server if there's a high enough percentage of oceania/asia players that sign up and are active
2) If you sign up, it is recommended that you visit the forums daily for announcements and tactics discussions however there is no activity requirement. The old structure for battles were battle days on Saturday and Sunday with the battles lasting half a day (noon to midnight edt as a baseline). Players would rotate in and out on a queue basis with no requirement on how long they had to play.
3) The previous point brings up another that hasn't been mentioned, that being the case when servers are full and the queue to play is long. Previously in IC, the sides would implement mandatory rotation after 2 hours to ensure most people got a chance to play. This issue will again be left up to the individual sides to decide on a rotation policy but it will still be overseen by management to ensure fairness. If there is high enough interest we can also discuss having multiple servers operating at the same time to accommodate everyone.
4) Realism mode in the past IC was a spot of contention. Of course back then that largely meant no death messages

I believe federov puts it best by placing the focus on mutators rather than a specific realism level to accomplish what the campaigns need. The spirit of IC is community and plenty of good natured "I totally ran you over evilhobo!" We will continue to harbor this kind of interaction and I feel that full realism mode might detract from the semi-casual nature of IC.
5) You sign up for a side at the beginning of a campaign and play for them for the duration. After the campaign ends, registration for a new one begins and you will have an option to play for whichever side you want. The servers will be password protected on battle days and possibly white-listed with users UIDs or Steam IDs to prevent disruption. Otherwise the plan would be to have the server open to the public on non-battle days to raise IC awareness.
6) I am open to moving back the start of the first campaign a couple weeks (especially since I get married sept 10

) if that will help to get things better organized and give players a better feel. The other option is to have a short "beta" campaign to test out rules and concepts while also allowing everyone to come up to speed on the game before throwing them into a full scale campaign. I would like a bit more discussion before I decide one way or the other.
