I can't really speak for any Hard+ difficulties, but for every 4 players I see on Normal/Beginner that make me plant my palm into my face with their hilarious flamethrower + berserker combinations there's at least one decent one who kinda knows what they're doing - and if not, they pick it up fast enough.
To be fair, they should really have some sort of training course for new players, once you play beginner for a while you see players getting into the habit of getting away with things. Whether it's playing a Firebug while rocking an AA12, not sharing money with new/recently resurrected players, or enraging Scrakes/Fleshpounds - these habits add up, and once they move onto greater difficulties - they can be potentially fatal for the team as a whole.
I've only had the game for what, a week or so, so I'm no expert or had a great deal of experience, but I've only seen one purposeful griefer in my time, and frankly, as long as new berserkers keep on getting killed by Clots I get my kicks, one way or the other >
