Started the single player campaign, Axis, and after firing the SMG through the first couple tests the hold left shift test started, but at that point I get stuck. For some reason the hold breath (left shift0 command doesn't respond so I'm stuck in that test (box). I have tried remapping the key and everything even restarting the game, but at that point I can not control my breathing to pass the section.

I was about to start this same topic. I have the same problem. Shift seems to work for everything else it's bound to, such as sprinting, but controlling breath doesn't work.
Out of curiosity, are you using a PS/2 keyboard, or USB? I'm using PS/2.
I noticed it was still prompting me too, but what I had to do was shoot one of the targets that was on the ground which I thought I had already shot.
If it says (top of screen) you haven't shot all the targets (like saying 3/4 or 4/5) then try shooting the ones on the ground.

Well I shoot the targets but they don't drop and it'll say I haven't shot any, like 0/4. Since I can't hold my breath it's like I can't knock the targets down.
I'm having the same problem. I hope someone will be able to help us, I really want to play this game.
I am also having this problem. I have tried binding the command to many different keys, including "Left Shift" as the tutorial states. No Go. Using USB keyboard.
Pretty sad that two entire campaigns are blocked from us because the 'hold breath' key binding is screwed up and there's no way to skip the tutorial.

Yeah this worked for me, still didn't solve the hold breath issue, but it did allow me to drop the targets and move on.

Using default bindings and the mysterious "H" key (a command which is not listed in the bindable commands) I was able to get through the tutorial mission.
It would be nice if whatever command "H" performs was listed as an option in binding, it sure makes targets easier to hit.
As it is, I guess there will be no breath-holding/zooming/whatever for me, because I'm not going to learn a different key layout just so I can access the H key.

uncheck disable sprint key zoom in the first options menu
Thanks for reminding me, I really dislike sprint zooming. Had it unchecked during the beta as well. I must of missed it when reconfiguring for the final release.
