You should buy it. The story and the atmosphere make it a must play.
Graphics need to be just as good as they need to be to make the game's atmosphere work and in that regard, Mafia could looks a lot worse and it would still be a must play.

EDIT: Oh, and I think it is possible to steer with the mouse. Its been a while so I could be wrong.

Truely, its a classic. The atmosphere in this game is second to none, shootouts are difficult but when you get used to them they absolutely rock!
And the story and morality of it all is just plain awesome!
There's a lot of driving around, (wich i personally didnt like so much) so you have to be prepared for that, but it's very great.
