if your fps get to low you won't see any blood, normally you see some spikes with blood covering the head. When a head gets blown of it just gets made smaller and blood goes over it. If your fps are below your wanted FPS you don't see alot of decals including blood. So you see what you're not supposed to see aka whats behind the blood. The tiny head.
It's the way the Unreal engine works, when you head-shot someone it shrinks their head really fast, same in all the UT games, you can see it if you slow the game speed down.
Yeah it only happened once. Probably an overheated hardware or something. It looks even weirder in motion: people walking around with weird laser beams coming out them.
(or maybe it's just my secret hax affecting the game. )
