When you use a catana in your hands as your primary weapon, your model (3rd person) shows that you have one additional catana sheathed on your side (you can see its handle). That's not really a bug. It's just something that was never implemented since the sheathed katana model is just one model attached to the player model. It's the same thing like you see with grenades. It will always show 2 grenades attached to the player even after they have used them.
