Console Instant death on Nightmare

So a new game started i turned around and went to the left of the car and went to collect the thing and died instantly.

Shows fine to me.
Tldr: player spawns at the begining of the nightmare (near blue camaro), runs to a tree stump where the collectible lies (left to the camaro) and just dies.
Its just instantaneous as if suicide command was used.

I have been having this happen as well - mainly on smaller maps like nightmare or descent.

Kittenmittens Quick question: Was this issue looked over and can we expect a fix on an upcoming update?

Because bizarrely on PC people did suddenly die on nightmare as well today.
1st death being on the beginning room, then in the morgue and then on a carnival room.
​​​​​And then final sudden death on boss wave.

Ukrainian server that for the most part runs fine, and this bug only happens on nightmare as far as i've seen.

I died during trader time yesterday someone dropped cage on me on monster ball and it killed me LMAO was annoying at the time, ive died from nothing a few times it's not just on nightmare had it happen on burning Paris , infernal realm, prison and the descent
