Is apperance changing only for heroes or for very high honor level or highly decorated status or both ?? 
Visual difference only for heroes

Well i dont really see anything diffrent between for example a raw recruit and a battle hardened soldier

Ive seen some people running around with highly dedicated, they appear to be much dirtier then your average soldier

It's hard to tell, because you unable to take a decent look on your own character for comparing.
At least there is no any visual difference between recruit and veteran imo. Yes, I saw some dirt on some soldiers, but that was it?
