Randomly and constantly being logged out of PSN or experiencing game crashes during gameplay when either completing objectives or obtaining collectibles. Normally after these kicks or crashes, the items or quests are still available to do, or they register as 'finished' and if a trophy is attached to them, it's not rewarded.
Chased an Apex Predator into another region (Prosperity Sands into The Gulf) and killed the AP, only to have it disappear and not reward with either the 'This Is My Movie' trophy or the XP, but the mission was 'completed'-- no do-over or respawn.
Still did not get completion trophies for two regions and have re-entered them numerous times.
Completion trophy for license plates collected failed, visibly have 100% of them in the menus.
